Description

The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple co-morbidities. This position works in coordination with multiple PCPs and serves as the central link to an identified population of patients.

Required Qualifications:

  • Coursework/Training: Graduation from an approved, accredited school of Nursing.
  • Washington Registered Nurse License upon hire.
  • 2 years of work experience in an RN role.
  • Work experience revolving around medications and IV solutions in terms of purpose, administration, side effects and contraindications and administers according to established standards and provider orders.
  • Experience managing complex patients in an ambulatory setting.

Preferred Qualifications:

  • Bachelor's Degree in Nursing.
