Float Primary Care Nurse (RN or LPN)

Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Work Model: On-site training for 3 months, then hybrid (1–2 days on-site weekly + monthly clinic meetings)

This is a unique opportunity to be part of a collaborative care team, working closely with providers to manage triage calls and messages, and support patient care coordination.
After completing a three-month on-site training period, you’ll have the flexibility to transition to a hybrid work schedule , with regular in-clinic collaboration.
