What You'll Do

In this rewarding position, you will:

• Conduct psychosocial assessments to identify emotional, social, and environmental strengths and challenges.
• Develop and implement a bereavement plan to support family members and caregivers.
• Communicate and interpret cultural and spiritual beliefs that may impact care.
• Educate patients and families in a way that meets their unique needs and overcomes barriers.
• Document assessments and interventions in patients' electronic medical records.
• Facilitate patient decisions regarding self-determined life care .
• Respond to indicators of imminent death and address family needs compassionately.
• Assist with referrals for community resources, including food, clothing, and financial assistance.

What We’re Looking For

To thrive in this role, you should have:

• Education: A Master’s Degree in Social Work.
• Licensure: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois.
• Experience: At least 1 year of experience in hospice care.
• Skills: Ability to work collaboratively within a team and demonstrate sensitivity to the needs of patients and families.

Working Conditions

Hours may vary based on patient census and program needs, allowing for flexibility in your schedule.
